Below is a listing of the Pack 198 forms we require to be filled out yearly along with a description of each form.
The Code of Conduct Contract is between Pack 198 leaders, parents, and Scouts to ensure that our Scouting program is successful and safe for all participants. This should be signed by each Scout and his or her parents/guardians. ALL parents/guardians are expected to sign the contract.
Safety is of the utmost importance for Pack 198, and as such, we want to ensure we protect our Scouts from being pictured or named when they should not be. All parents are expected to fill out and sign this release form that instructs Pack 198 leaders on how their Scout(s) images and names can be used.
Pack 198 is a very active group, and we need all the help we can get! The Parent Talent Survey helps leaders know what parents' interests and talents are so we can better fit you with volunteer opportunities.
In order to keep our records up-to-date, Pack 198 requires all families to fill out the Membership Renewal Application. For new or transferring Scouts, membership applications should be filled out through the online BSA Youth Application.
